This form is for patients who need a form signed for their association, please email us the form with YOUR portion filled out and we will complete our portion. PLEASE NOTE THIS OPTION CANNOT BE PURCHASED UNLESS OPTION 1 HAS BEEN PRE-PURCHASED. Once payment is received please allow 24-48 business hours.

Fine Print For Letters:
There is no refunds for ESA/Service animal Letters AFTER THEY HAVE BEEN SENT!
There is no name changes once letters have been issued to the corresponding client
We do not back date any letters past the day of application approval
We are not responsible for any mistakes made during your application process if an email is not received prior to your letter being delivered
We do not issue one letter for multiple animals. Each animal will have its own corresponding letter for each task they perform separately
We are not responsible if your letter is denied once it has been issued
We are not held liable if your letter is needed by your association or airline by a specific timeframe. Our letters are issued within 48 hours of your letter being APPROVED (NOT SUBMITTED).
